A6 issue resolved.
Point to note it’s always used with scell intra Freq neighbour cells only not valid for inter.

Correction here for s-measure statement: serving level must be above s-measure for Intra freq MR.
Mgap not needed for intra freq.
If s-measure is -60 then UE can only measure intra freq cells if its RSRP is less than -60 (-61dBm and so)
But if serving RSRPvisb -55 dBm and mgap is configured then UE can measure both intra and inter.
You can test above in lab.
I tried it’s not sending intra Freq MR.
With what scenario?
S-measure -80 and serving rsrp intra and inter Freq -70 with mgap configured then ue fail to send MR.
We need to cross -80, i.e. -81.

Yes once I lower s_measure, i.e. -60 immediately UE send A5.
So in summary: as long as the serving RSRP is greater than S_Measure, it does not measure intra Freq Nbr.
But I didn’t understand your 2nd statement. Are you saying that If A2 threshold is -54 dBm, then measurement gap will initiate…?
