MME has not triggered the eNodeB to set up the UE context yet meaning

Hello Experts.

L.UECNTX.Rel.MME.AbnormRel.Init counter is incremented by 1 each time the following conditions are met:

  1. The MME sends a UE CONTEXT RELEASE COMMAND message to the eNodeB to release the UE context when the MME has not triggered the eNodeB to set up the UE context yet.

What is meant by when the MME has not triggered the eNodeB to set up the UE context yet?
How the Ue context was setup in the first place.
Is it related to fraud?
